Caregiver handoff: share the last feed and diaper change

A practical handoff checklist for parents, partners and caregivers, with a printable record of what happened and what is still open.

Start with the latest completed events

Record the last feed’s time and type, the last diaper change and any useful observations. Use the actual event time. A pumping entry is separate from a bottle the baby has taken; make that distinction clear during handoff.

Say what is done and what is still open

Write who is taking over and note any unfinished task or question. Keep instructions separate from the event history, and follow the care plan agreed with the parent or clinician. The template does not create feeding or medicine instructions.

Choose one shared record

Agree where updates go, and check access before the handoff. Avoid keeping two competing versions in messages and a notebook. In Nuzzle, caregivers you invite can see the same feeding and diaper record; use the guide as a checklist whether you choose the app, paper or a spreadsheet.

A blank record to start with

Print this page using your browser, or download the CSV and open it in a spreadsheet. Add rows as you need them. The template is a record, not a feeding schedule or a medical assessment.

On a small screen, scroll the table sideways to see every column.

Nuzzle keeps a record. It isn't a medical device and doesn't give medical advice. For anything about your baby's health, talk to your pediatrician.

Keep the record together in Nuzzle

On your iPhone, Nuzzle records nursing, bottles, pumping and diaper changes. Invite your partner or another caregiver to see the same record. Purchase options and local prices are shown in the app.
