Diaper change log: a simple shared record

Keep a record of wet, dirty and mixed diaper changes with a blank printable table and CSV for caregivers.

Use the same labels every time

Write the date, time and whether the diaper was wet, dirty or both. Agree on these labels with anyone else keeping the record, so a mixed change is not accidentally counted as two separate changes.

Add observations in plain words

If a detail matters, describe what you observed rather than attaching a diagnosis. Mark a late entry with the time of the change. Keep notes short enough that the next caregiver can read them quickly.

Bring the record to a conversation

A shared log can help you explain recent changes to your pediatrician. Counts alone are not a medical assessment, and this template does not provide a normal-count threshold. If you are concerned about your baby, seek medical advice rather than waiting for the log to show a pattern.

A blank record to start with

Print this page using your browser, or download the CSV and open it in a spreadsheet. Add rows as you need them. The template is a record, not a feeding schedule or a medical assessment.

On a small screen, scroll the table sideways to see every column.

Nuzzle keeps a record. It isn't a medical device and doesn't give medical advice. For anything about your baby's health, talk to your pediatrician.

Keep the record together in Nuzzle

On your iPhone, Nuzzle records nursing, bottles, pumping and diaper changes. Invite your partner or another caregiver to see the same record. Purchase options and local prices are shown in the app.
