Sleep-space observation checklist

Look at the sleep space, then mark what you observe. Use this as a conversation starter with everyone who cares for your baby.

For babies under 1 year. Based on US NICHD guidance, not a complete inspection or medical advice. This cannot certify a product, predict SIDS or guarantee safety. Ask your baby’s clinician about individual needs and follow local product standards.

Mark each observation. The guidance stays visible whatever you choose. Unanswered and uncertain items remain on your follow-up list.

Start every sleep on the back

Place baby on their back for naps and nighttime sleep, including babies with reflux.

Firm, flat and level

Use a mattress and infant crib, bassinet or play yard meeting applicable safety standards. No incline, hammock shape, wedges or positioners.

Only a fitted sheet on the mattress

Keep pillows, loose bedding, blankets, toys and bumpers out. Do not use weighted blankets, weighted swaddles or other weighted items.

A separate space made for baby

No adult bed, couch or armchair, alone or shared with adults, children or pets. Return baby to their own space after feeding or comforting.

Head and face uncovered

Avoid overheating. Choose clothing for the room temperature, remove indoor hats and watch for sweating or a hot chest.

Smoke- and vape-free surroundings

Keep baby’s home, car and other surroundings free from smoke and vaping.
